Player Evaluation Form U-16

Please use the following criteria to assign player ratings (most players are 5, 6 or 7 players):
9 = Player has the ability to take over a game and completely dominate it, from any position
8 = Player has excellent skills in all disciplines of the game and can play any position
7 = Player can play most positions and has good skills
6 = Player plays 2 positions very well with adequate skills
5 = Player plays 1 position well, tries to learn other positions, attentive to instructions
4 = Player tries hard, enjoys playing but struggles at times
3 = Player seems to lack interest, doesn't seem to want to be there, stands around
(- = Player left AYSO program)

If the player was a goalkeeper, provide a separate rating for that position (6 = adequate; 7 = good; 8 = excellent) - you only need to rate players who played that position. Don't forget to rate goalkeepers as regular field players, too.
