Player Evaluation Forms have been created to help form well-balanced teams next season. All ratings are to be kept confidential. The only people who will see this form when you (the coach or assistant coach) are done, are the Regional Commissioner, the Registrar and the Division Coordinator.
Assistant Coaches, please fill out the top 3 lines and the bottom 4 questions of this form!

Select the appropriate form: U5, U6 and U-8, U-10 through U14 or U-16, fill out the information and click submit before June 1, which will e-mail the form to the RC.
And remember: no evaluation, no trophies! Trophies will be available only on Tuesday June 7, during the parent information evening, at the Senior Center, 7-9 pm. You will receive your trophies in return for 2 signed copies each, of your (and your assistant's) volunteer form for next season.

Balanced teams:
By July 4 the RC (Regional Commissioner) has to place the uniform order for the next Fall/Spring season. That means that players registered by that date, plus a number of potential players, will be scheduled for each team, which stays together for the next Fall and Spring. Based on experience, X-number teams will be created per boys or girls age group, using e-AYSO and experience from Division Coordinators. eAYSO will balance the teams based on the ratings (0-9) given by the coach of the previous season, meaning an equal average per team, whereby unrated players have a "2" rating. This is not perfect, since each evaluation is subjective (in the eye of the beholder); there will be players that were not rated by a coach; there will be new (unrated) players who may or may not be "high rated players"; players may develop ("wake up") over the summer; players may be switched around based on certain requests (if feasible); last minute or during the season additions/deletions etc.
